Greetings:
I’ve acquired several Sacramental Certificates(Baptism, Communion, and Confirmation).
The original Church, Holy Spirit Church(my Baptism 1957 and no longer there), was merged with 2 other churches forming the Parish of Saint Monica.
NOTE: All of the Sacramental Records from the other churches are kept at Saint Monica’s.
The certificate from the "Parish of Saint Monica " has the name of the original church at the top of the certificate.
I’m looking to create source type - either as Church Issued Certificates (collection) or Church Issued Certificates(single). The source name as either
The Parish of Saint Monica - [Sacrament] or The Parish of Saint Monica - Sacramental Certificates;
Or, The Parish of Saint Monica (Holy Spirit Church);
Or, Holy Spirit Church
If I understand what you are saying, either would work for all 3 Sacraments-- are the certificates just for your family or for a group of neighbors? Think that would be the key as to which one—that and who you want to list as the owner of the documents-- you personally who would pass this down to the next generation like a family bible or the Church-- either way the Parish of Saint Monica is the repository and I would add a date saying as of 2023 as who knows 5 years from now it could be merged again BUT you also need to make sure that you include that these were from the Holy Spirit Church originally and a date range–believe me , it is no fun trying to find your uncle’s Baptism or 3rd g-grandparent’s marriage and death info on microfilm when there are tons of Churches in the area…
The Certificates are for my father and siblings. The date range, could that be from the time of the merger, or, from time I received them, 2012 from the parish?
This is a starter, see attached source. I noticed the Footnote does not match EE, so I went in a changed it’s I will for the Short too. I created a Source for each of the Sacramental Records(Communion, Confirmation, and Marriage).
Looks pretty good to me-- as for the date range, you have the date when issued as well as the merger of the Churches-- I was thinking you MIGHT want to include a range of dates for the certificates themself-- such as yours was 1932-- so say your Dad’s was from 1912 and you had a much younger sister Baptized in 1949-- date range would be 1912-1949…
The only other thing that I MIGHT do differently is the Church name as you told us that this was originally Holy Spirit Church and you do mention the merger BUT if I didn’t know abt the merger, I would probably assume it was Saint Monica-- think I would put Church as The Holy Spirit now Saint Monica Parish
Why do you have Our Lady Star of the Sea ( Holy Spirit) on there was there a name change as well???
As for: Why do you have Our Lady Star of the Sea ( Holy Spirit) on there was there a name change as well???
Before the mergers to create The Parish of Saint Monica, there was a merger prior to it.
Holy Spirit was merged with Our Lady Star of the Sea(OLSS); later OLSS was merged with the other churches mentioned to create The Parish of Saint Monica.

When we identify the “parish” and “church” we say, Holy Spirit Church at Parish of Saint Monica.
So, in Master Source, "The Parish of Saint Monica(current) to Church as “Holy Spirit now Saint Monica”.
Therefore, I need to create another source for OLSS which is now part of Saint Monica.

I don’t see a range in the source, what field would the range go into?
For example, the date the person was baptized (from the example above, is my dad), 1932. The range 1932 - 2015, date of the merge. Is that the idea?
